The information provided in this online request form is being gathered by the Treasury Board of Canada Secretariat (TBS) in accordance with section 29.2 of the Financial Administration Act for the purpose of facilitating the receipt and delivery to participating institutions of requests submitted under Access to Information and Privacy (ATIP) legislation. Failure to provide this information will result in the inability to use the ATIP Online Request Service to submit a request for access.

The information you will be providing in this online request form will be held temporarily on servers hosted by TBS. Once requests are re-routed to and received by the institution selected by you, the applicant, the information will be deleted from TBS servers after 2 years.

The information relating to online requests will then be retained by the institution responsible for processing the request and will be kept for the period of time identified in standard Personal Information Bank PSU 901 (Access to Information and Privacy). It may only be used or disclosed in accordance with sections 7 and 8 of the Privacy Act.

The personal information you provide in this form will only be made available to employees who require it in order to process and respond to your request. Under the Privacy Act, you have the right to request access to your personal information held by a government institution and to request corrections, should you believe the information held by the government institution contains errors or omissions. If you wish to avail yourself of this right, you must contact the ATIP Coordinator of the institution.

Payment information will be redirected to a secure and trusted third-party website (Moneris Solutions) where your payment will be processed. Your name, credit card number, expiry date and security code (CVV2) will be collected in order to process your payment.

To ensure compliance with the Directive on the Social Insurance Number (SIN), individuals are asked not to provide their SIN during the online application process.

All of the information you input into this online request service will be encrypted, including any documents you choose to attach in support of your request; the latter will also be scanned for viruses. If you have concerns about the confidentiality of the transmission of information via this online tool, you may send your paper application and/or any necessary supporting documents by mail directly to the appropriate government institution.

Security Warning: If you are using this service on a shared computer in a public area (i.e. public kiosk or library), do not leave the computer unattended while accessing this service. Before leaving the computer, it is also important that you fully log out of the application, clear your browser's cache and close down your browser. These precautions will ensure that no one else can access any personal information you may have entered.
